Many documents will be needed and I will have to ask you many questions. This will be a partnership. I have attached two documents to this email that you will need to review. One is required federal disclosures and the other is my internal disclosure requirements. 

 

I pride myself on being less expensive, while also being more accessible than many of my competitors. Please call me anytime you have any questions. 

 

Please read the two documents attached above before proceeding. 

 

Here is a short list of what we'll need to get started. I'll need documents for all filers. When it refers to "filing date," it refers to the day we file for bankruptcy. 

 

  1. bank statements for the 3 months prior to filing date: this includes: checking, savings, and money market accounts, mutual funds and brokerage accounts.
  2. last three pay advises before filing date - and one six months prior to filing date.
  3. mortgage or rental documents - especially the deeds to any property you own.
  4. copies of your driver's license and social security card. 
  5. income taxes for last two years. 
  6. all three credit reports (Equifax, Trans Union, and Experian) - there is a $30 fee if I have to get them myself. 
  7. copies titles to property that has a title: including, but not limited to, automobiles, boats, motorcycles, trailers, and mobile homes.
  8. copies of any personal property leases, including motor vehicle leases.
  9.  title documents to all real estate in which you have an interest, including deeds, registered land certificates of title, land contracts, or leases.
  10.  closing statements for any interest in real estate sold or conveyed by you within the year preceding the petition filing date.
  11.  all life insurance policies owned by the debtor, and evidence of the cash surrender value and the beneficiary.
  12.  copies of any separation agreements or decrees of dissolution or divorce entered into or granted during the twelve (12) months prior to the petition filing date.
  13.  all documents evidencing your interest in any retirement account.
  14.  copies of any security agreements, and financing statements.
  15. copies of stock certificates, bonds, credit union accounts, and other evidence of investments or savings.
